HIPAA Notice of Privacy Practices
Last updated: October 17, 2026
This notice describes how medical information about your child may be used and shared, and how you can get access to that information. Please read it carefully.
Effective date: October 17, 2026
Professional Healthcare & Behavioral Services Inc is required by law to protect the privacy of protected health information, to give you this notice of our legal duties and privacy practices, and to follow the terms of the notice currently in effect.
"Protected health information", shortened to PHI below, means information that identifies your child and relates to their health, their care, or payment for that care.
This notice is about health information. How this website handles what you type into it is a separate thing, described in our Privacy Policy.
How we may use and share your child's information
Without asking you first
The law lets us use and share PHI for these purposes:
- Treatment. To plan and deliver care, and to coordinate with others involved in it, such as your child's pediatrician, school team, speech therapist, or occupational therapist.
- Payment. To check benefits, get authorization, and bill your health plan for services.
- Health care operations. To supervise our clinicians, review the quality of our care, train staff, and run the practice.
Other uses the law permits or requires
- When a law requires us to report something.
- To report suspected abuse, neglect, or exploitation of a child or a vulnerable adult, which Florida law requires us to do.
- To prevent a serious and immediate threat to the health or safety of your child or someone else.
- For public health activities and health oversight agencies.
- In response to a court order, subpoena, or other lawful legal process.
- For workers' compensation claims, where applicable.
- To a coroner, medical examiner, or funeral director.
- For research, only where an approved review board has authorized it and privacy protections are in place.
Only with your written permission
We will ask for your written authorization before we use or share PHI for anything else. That always includes marketing, the sale of PHI, and most psychotherapy notes. You can take that permission back in writing at any time, which stops any future use, though it cannot undo something already done while it was in force.
Your rights
You have the right to:
- See and get a copyof your child's record, including an electronic copy where we hold it electronically. We will respond within 30 days. We may charge a reasonable, cost-based fee for copies.
- Ask us to correct information you believe is wrong or incomplete. If we deny the request, we will tell you why in writing, and you may file a statement of disagreement that we keep with the record.
- Get a list of disclosures we have made outside of treatment, payment, and operations, covering up to the six years before your request.
- Ask us to limit what we use or share. We are not required to agree, with one exception: if you pay in full out of pocket for a service, you can require us not to share that information with your health plan, and we must comply.
- Ask us to contact you a different way, for example at a specific phone number or address. We will accommodate reasonable requests and will not ask why.
- Get a paper copy of this notice at any time, even if you agreed to receive it electronically. Just ask.
- Be told if there is a breachthat compromises your child's unsecured PHI.
- Choose someone to act for you.A parent, guardian, or person with medical power of attorney can exercise these rights on your child's behalf, once we have verified their authority.
Our duties
- We are required by law to keep PHI private and secure.
- We must give you this notice of our legal duties and privacy practices.
- We must follow the terms of the notice currently in effect.
- We must let you know promptly if a breach occurs that compromises your child's unsecured PHI.
