Privacy Policy
Last updated: October 17, 2026
This policy explains what Professional Healthcare & Behavioral Services Inc collects through this website, why we use it, and the choices you have. It covers this website only. How we handle health information once a child is in our care is covered by our HIPAA Notice of Privacy Practices, which is a separate document and is not replaced by this one.
What this website collects
This is a static website. It does not use advertising trackers, it does not sell information, and it does not build a profile of you.
Information you choose to give us
If you use the request form, you may enter your name, your phone number, your email address, your child's age range, your insurance plan, and anything you write in the message box. You decide what to put there.
The form never asks for a diagnosis, and you should not send medical details, test results, or records through it. A web form is the wrong place for that, and we do not need it to start a conversation with you.
Information collected automatically
Our hosting provider, Cloudflare, processes limited technical information needed to serve and protect the site, such as the requesting IP address and basic request data. That is standard web hosting and abuse prevention. We do not run analytics, advertising pixels, or cross-site tracking on this site.
How the request form currently works
We are being specific here rather than vague, because you deserve to know exactly where your words go.
The request form on this website does not yet transmit anything to us. When you submit it, the page does not send the information anywhere. It asks you to call us on (786) 432-7113 instead. Nothing you typed leaves your own browser, and we never receive it.
We chose this over a form that looks like it worked but quietly went nowhere. When we connect the form to a real, protected destination, we will say on this page exactly where submissions go, who processes them, how long they are kept, and under what agreement. Until then, the phone is the way to reach us.
Why we use the information you send
- To answer your question and call you back.
- To check whether your insurance plan covers services with us.
- To schedule an assessment if you decide to go ahead.
- To keep records we are required by law to keep.
We do not use what you send for marketing lists, and we do not sell or rent it to anyone.
Who we share it with
We share information only when there is a reason to, and only with:
- Service providers that run the parts of our operation we do not run ourselves, such as website hosting and email delivery. They may only handle information to do that job for us.
- Your health plan, when you ask us to check your coverage or bill for services.
- Anyone you tell us to, in writing.
- Authorities, when a law, a court order, or a duty to report abuse or an immediate safety risk requires it.
How we protect it
This website is served only over an encrypted connection (HTTPS), so what travels between your browser and the site cannot be read in transit. Access to our inboxes is limited to the people who need it, and is protected by individual accounts and multi-factor sign-in.
No website and no inbox is perfectly secure. That is true everywhere, and it is the honest reason we ask you not to send medical details by web form or ordinary email.
How long we keep it
We keep an inquiry only as long as we need it to answer you or to meet a legal obligation. If an inquiry becomes part of a child's intake or clinical record, it is then held under the rules in our HIPAA Notice of Privacy Practices and the retention periods Florida law sets for health records, not under this policy.
Your choices and your requests
You can ask us at any time to:
- Tell you what information we hold about your inquiry.
- Correct it if it is wrong.
- Delete it, where we are not required to keep it.
- Stop contacting you.
Call (786) 432-7113 or email [email protected] and say what you want. We will answer in plain language and will not make you justify the request.
Children
This website is written for parents and caregivers, not for children. We do not knowingly collect information directly from a child through this site. Information about a child comes from the adult responsible for them.
Emergencies
This website and our inboxes are not monitored for emergencies. If a child is in danger or needs urgent help, call 911 or go to the nearest emergency room. For a mental health crisis, call or text 988.
Changes to this policy
If how we handle information changes in a way that matters, we update this page and change the Last updated date at the top. The date tells you when it last really changed.
